That's a Swampire. It's a feared health-sucker that hides in the darkness. Max HP is 20, Attack is 6, and Defense is 0. It sucks health from its prey to add to its own HP. And that...totally grosses me out like you wouldn't believe. If you let it feast on you, its HP will get really high. Focus your attacks on it!

Swampires are a powerful variant of the Swooper and the strongest of its relatives found in Paper Mario: The Thousand-Year Door. Its name is a portmanteau of the words "Swooper" and "vampire", referring to its batlike nature and health-sucking ability. Swampires have notably high HP, tied with several enemies for the second highest HP of any non-boss enemy after the Dark Koopatrol. Like other HP-sucking enemies, such as Fuzzies, they replenish lost health equal to the amount of damage dealt to their attack's target. However, this ability is disabled by the Electrified status, incapable of damaging the affected target regardless of the Swampire's attack power.
Swampires cannot be battled during the main story of Paper Mario: The Thousand-Year Door. They are only encountered after floor 90 in the Pit of 100 Trials.
