During the battle against Dark Fawful in the [[Conference Hall]], Bowser's [[punch]]ing and [[Flame Breath]] attacks have no effect on Dark Fawful due to his helmet, though some of his [[Brawl Attack|Special Attacks]] can damage him. One of Dark Fawful's attacks involve his helmet growing teeth and latching onto a chandelier lamp in the room to swing himself into Bowser (which is not present in the remake), which first needs to be countered with a few punches, and then [[Shell defense|ducking in his shell]] to avoid getting hit from behind. At times, the helmet will recover 300 of Dark Fawful's HP (500 in the remake) by exhaling purple gas at him. It is also revealed that the Headgear's nozzle can morph into a kind of basket, inside of which [[Mario]] or [[Luigi]] can be trapped and drained of their [[HP]]. While Fawful is fighting Bowser, he may use his headgear to avoid some of Bowser's attacks or he can revive some of his own HP. The headgear also morphs it's nozzle into what looks like teeth, allowing Fawful to grab onto objects and swing from them. At some point, the headgear will fly above Fawful's head on the top screen, where Bowser can use his vacuum ability to suck it up into his body, where the bros. will fight it. After taking a certain amount of damage, the headgear will explode and cease to exist, making its last appearance.

The Vacuum Helmet, also known as the Headgear, is a device used by Fawful in Mario & Luigi: Superstar Saga, Mario & Luigi: Bowser's Inside Story, Mario & Luigi: Superstar Saga + Bowser's Minions, and Mario & Luigi: Bowser's Inside Story + Bowser Jr.'s Journey. In Mario & Luigi: Superstar Saga and Mario & Luigi: Superstar Saga + Bowser's Minions, Fawful uses the headgear to suck up Princess Peach's voice before the onset of the game.
