When Mario or Luigi drinks it, he gains a permanent 4 point boost (2 in the remake) to his BP stat. When supplied to E. Gadd upon first making the blend, he humorously ends up being forced to consider going to the restroom (and realizing he forgot to build restrooms inside Starbeans), resulting in Luigi sucking up E. Gadd with the Poltergust 3000.
