Bonus Medals are flashing, rainbow-colored Star Medals found in Remix 10 mode of Super Mario Run. Three Bonus Medals are found in each normal course; Special Remix Courses and Secret Courses have five Bonus Medals. Due to the short length of the courses, Bonus Medals are often found grouped together, although they are more spaced apart in some cases. Their positions tend to correlate with the color coins that appear in the other modes of the game. In VS Boom Boom and VS Bowser, Boom Boom, fake Bowser, or Bowser hold all three of the Bonus Medals and grant them to Mario when defeated.

After completing a course, the Bonus Medals the player collected are displayed. Uncollected Bonus Medals remain in outline form. They are ordered to correspond with the linear order appear in the course, much like color coins. Bonus Medals held by Lakitu are last in the this order.

Dying at any point makes the player drop all of the Bonus Medals obtained in that course. If Mario is defeated in VS Boom Boom or VS Bowser, he loses his progress in the round, including all Bonus Medals. Successfully clearing every course in a round of Remix 10 adds extra two Bonus Medals to the player's counter at the end of the round.

If the player collects all Bonus Medals in a course, they receive a "Perfect!" result, and the space on the map becomes rainbow colored. Achieving a "Perfect!" for every course in a round adds extra three Bonus Medals to the player's counter.

Collecting Bonus Medals fills up the gauge for the Bonus ? Block, which can unlock rare items for Kingdom Builder. Once it is full, the gauge resets and the player is then able to use the Bonus ? Block one time. The player also receives extra Bonus Medals for reaching a certain milestone in Remix 10.

If the player finishes the last course in an round with their coin counter showing a multiple of 11 (22, 33, 44 etc.), an icon of a Bonus Medal on a cloud will appear on the map screen over the first course of the next round for 15 minutes. This icon is tied to a short bonus course containing many coins and five Bonus Medals. On rare occasions, when the player starts a round with this course, the icon will begin to sparkle; when this happens, every coin in the course is instead a Bonus Medal, allowing the player to obtain a large amount of Bonus Medals. The normal display for Bonus Medals is replaced by a counter in this variation. Collecting any number of Bonus Medals is treated as a "Perfect" result.

In a special limited-time event, the Bonus Medals collected are multiplied by 1.2. Here are the following dates that the event was held on:
