Gahaha Daiō is Bowser's fifth alter-ego in the KC Deluxe series, during the events of the Dr. Mario. Costume-wise, Bowser just wears a red cape (probably the same from the Super Mario Bros. 3 events), a wrestler-like belt with the characters「大王」(daiō, "king") written on it, and a smiley face on the forehead which, from chapter 2 onwards, it changes expression to match Bowser's.

Dressing up as a Dutch flower girl, Gahaha Daiō infiltrates the Mushroom Kingdom Ikebana Expo, then reveals his disguise and unleash three Dokudoku Pakkun Flower that release the germs he created, the Gahaha Virus, which causes everyone infected to laugh until death. Mario manages to not get infected, and while investigating, discovers that a village in the Kinoko Yama is safe from the Virus. Gahaha Daiō follows him, intending to stop him and then take over the last village. While there, Gahaha Daiō disguises himself as an old lady and offers Mario an apple filled with ten doses of Gahaha Virus. Peach and Daisy end up eating it instead, speeding up the effect of the Gahaha Biyō. Gahaha Daiō then sics the Dokudoku Pakkun Flower onto Mario and the villagers, but they discover that the divine Miracle Kinoko is impervious to the Virus and repels the plants, so Mario quickly creates an antidote in capsules form, saves the princess, then throws the cure in the plant's mouth, causing them to crumble like stone onto Gahaha Daiō. After that, the self-proclaimed Dr. Mario cures the entire Kingdom.

The next day, Gahaha Daiō manages to find the last remaining Gahaha Virus in Mario's house and uses his magic to decompose it into three larger Viruses and making it immune to the cure. The Viruses infect Luigi (mistaking him for Mario). At the Mushroom Kingdom Hospital, Gahaha Daiō sends Dr. Mario a message: Luigi has only six hours left to live, and if Princess Peach agrees to marry him, he might save his life. Peach refuses and collaborates with Dr. Mario, and the two travels with a shrinking Marine Pop inside Luigi's body. During their second trip, after they reconstructed the cure, Gahaha Daiō sneaks inside the ship and, before they can face the Viruses in the brain, steals Dr. Mario gun, ship, and Peach. By reading a Dr. Mario guide, Luigi's mind is able to manifest it in his brain, helping Mario in clearing the Viruses. At that, Gahaha Daiō drives outside the body to burn the magazine, then he proceeds to scare Luigi with his impeding doom, causing his brain to black out and darkening the place, and making it difficult for Dr. Mario to hit the Viruses. To stop him, Peach agrees to marry him, and when he goes for a kiss, Peach dodges it and Gahaha Daiō slams his head onto a wall, knocking himself out. After Mario manages to defeat and capture the Viruses, the three escapes Peach and Luigi's fury by hiding inside Bowser, infecting him.

On Sports Day, Gahaha Daiō and the three Viruses attack the stadium, where Dr. Mario is working at the medical stand. He challenges him to a VS. game of Dr. Mario, if he loses, Gahaha Daiō will release a party ball filled with Gahaha Viruses onto the stadium, while the three enlarged Viruses will avoid people from evacuating. Mario accepts the challenge and, aided by his friends, is also winning. To stop that, Gahaha Daiō uses his smiley face on his forehead to brainwash Luigi, Peach, and Kuribō, dressing them as Viruses and making them hinder Mario's capsule throws. Fortunately for Dr. Mario, the crowd now helps him and he manage to win. Gahaha Daiō still tries to activate the party ball, but the remote does not let him, since he lost, so he orders the Viruses to manually open it. But Mario turns giant by eating a bunch of Super Mushrooms and punches the Viruses and the party ball away. Taking advantage of this, Gahaha Daiō hids himself in a fireworks cannon, but Luigi ignites it and he blows up in the air.
