World 5 is a cloud-themed world in Dr. Mario World. It is available since the initial version of this game.

When an area is cleared, Spinies, Paratroopas, and Lakitus appear. The viruses featured in the regular stages are red, yellow and blue.

The objective of this stage is to collect all purple coins. In this stage, eight rows of four purple coins are joined in the middle from the second to ninth rows. Viruses (normal, half-frozen and fully-frozen) are located above each row of coins.

Purple coins are unlocked in the following sequence:

  1. The coins in the second and third rows.
  2. The coins in the eighth and ninth rows.
  3. The coins in the sixth and seventh rows.
  4. The coins in the fourth and fifth rows.

Prior to version 1.3.5, the stage layout was different. The following changes were made:

  • The viruses in the fourth and fifth rows are changed from normal to fully-frozen.
  • The viruses in the sixth and seventh rows are changed from normal to half-frozen.

The capsule amount is decreased from 35 to 23 since version 2.0.0. Besides that, the unlocking order of purple coins have changed so that the coins in the eighth and ninth rows are unlocked second (previously last) while the coins in the fourth and fifth rows are unlocked last (previously second).

The capsule amount is increased from 32 to 35 since version 1.3.5. Besides that, this stage is otherwise the same.

The last stage of the first area, the objective of this stage is to eliminate all viruses. In the beginning of the stage, there is a group of Brick Blocks surrounding three isolated exploders and a group of viruses, and clouded viruses are at the top-right side. As the stage scrolls, ten viruses inside bubbles, four half-frozen viruses and two clouded viruses are found in addition to twenty normal viruses, as well as a shell.

This stage will scroll for one line each time enough objects are cleared and will stop after 9 lines are introduced.

When cleared, the rest of the first area in the world map gets cleared of viruses. Players also obtain 500 coins upon completing this stage.

Prior to version 2.0.0, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 56 to 50.
  • The row of Empty Blocks visible after the stage scrolled all the way is removed, moving the stage objects up one line.
  • The Brick Blocks in the beginning within the second and third rows and second and third columns are changed to regular viruses.
  • A bubble virus is added in the third column in the row that is visible after the stage scrolls up once.
  • The shell visible after the stage scrolls up three times is no longer half-frozen.
  • The second and fourth Brick Blocks visible after the stage scrolls up seven times are changed to normal viruses.

  • The cages are moved up one row, and subsequently, the Brick Blocks in the tenth row are removed.
  • The lock blocks have their positions changed: the yellow lock block moved from the first row to the fifth row, the blue lock block moved from the sixth row and fourth column to the fifth row and the second column, the red lock block moved from the eighth row to the ninth row.
  • A hidden virus is added where the blue lock block used to be, relative to the changing position of the letters.

The final stage is a scrolling stage that requires all viruses to be eliminated. In this stage, various Floatie Viruses are found in the beginning, along with Brick Blocks and viruses in barrels throughout the stages. Bubble viruses appear eventually, and later ice fans start to appear. Exploders later appear, and a couple of half-frozen viruses one line after that. A line of regular viruses are found near the top, with the top containing a line of Empty Blocks. This stage will scroll for one line each time enough objects are cleared, and will stop after 10 lines are introduced.

Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 6. Upon completion, players earn a free Staff Ticket.

A couple of viruses are found in unbroken barrels.

Starting from version 1.2.0, completing this stage awards the player 500 coins.

Within a 65 second time limit, players need to eliminate all viruses. In this stage, viruses are lined up in a row, with shells found in three of the rows. All rows except for the topmost row has viruses found in partially damaged barrels. Separating each row of viruses are colorful crates, each with a gap. The first row of colorful crates has a shell in the gap.

Completing this stage awards the player 1000 coins and a Staff Ticket (3 Hearts prior to version 1.2.0).

The objective of this stage is to eliminate all viruses. In this stage, there are three sections separated by two separate rows of Empty Blocks. Each section has a row or Brick Blocks with viruses between them and the row of Empty Blocks. In addition to normal viruses, the middle section contains floatie viruses and viruses inside bubbles, while the top section contains clouded viruses and dust viruses (both types also contain floatie viruses).

This stage will scroll for one line each time enough objects are cleared or enough capsules are introduced to the stage, and will stop after 3 lines are introduced.

Completing this stage awards the player 1000 coins and Dr. Toadette.
