World 5 is a cloud-themed world in Dr. Mario World. It is available since the initial version of this game.

When an area is cleared, Spinies, Paratroopas, and Lakitus appear. The viruses featured in the regular stages are red, yellow and blue.

This stage introduces clouds, which shroud objects behind it and will temporarily become transparent as to reveal its contents. Any object behind a cloud behaves normally, but destroying that object will cause the cloud to disappear. The objective of this stage is to eliminate all viruses. In this stage, there are two groups of eight viruses shrouded in clouds above two rows consisting of Brick Blocks and viruses.

The top portion contains two rows of viruses containing three in the middle at the top and a full row at the bottom, while the bottom portion contains a row of viruses.

The objective of this stage is to collect all coins from Coin Blocks. In this stage, there are four rows of seven Brick Blocks, and between each of them are viruses in the odd-numbered columns. The topmost row has viruses in the leftmost, center and rightmost sides. The Brick Blocks that contain coins are the even-numbered columns for the first and third rows and the odd-numbered columns for the second and fourth rows.

On the leftmost and rightmost columns, there are lock blocks between Empty Blocks on the top and bottom, as well as some viruses below the Empty Blocks. At the two topmost rows, there are two groups of viruses (some are caged) separated by a column of Brick Blocks.

The objective of this stage is to collect all purple coins. In this stage, eight rows of four purple coins are joined in the middle from the second to ninth rows. Viruses (normal, half-frozen and fully-frozen) are located above each row of coins.

Purple coins are unlocked in the following sequence:

  1. The coins in the second and third rows.
  2. The coins in the eighth and ninth rows.
  3. The coins in the sixth and seventh rows.
  4. The coins in the fourth and fifth rows.

Prior to version 1.3.5, the stage layout was different. The following changes were made:

  • The viruses in the fourth and fifth rows are changed from normal to fully-frozen.
  • The viruses in the sixth and seventh rows are changed from normal to half-frozen.

The capsule amount is decreased from 35 to 23 since version 2.0.0. Besides that, the unlocking order of purple coins has changed so that the coins in the eighth and ninth rows are unlocked second (previously last) while the coins in the fourth and fifth rows are unlocked last (previously second).

The capsule amount is increased from 23 to 29 since version 2.1.0. Besides that, this stage is otherwise the same.

  • Four of the Brick Blocks are removed: the first and last ones in the first row, the last one in the third row and the last one in the fifth row.

The objective of this stage is to eliminate all viruses. This stage is shaped like a Fire Flower, which is formed with normal viruses, floatie viruses and Brick Blocks. The Brick Blocks between the floatie viruses contain hidden normal viruses, while the "stem" contains hidden floatie viruses.

The capsule amount is increased from 32 to 35 since version 1.3.5. Besides that, this stage is otherwise the same.

The last stage of the first area, the objective of this stage is to eliminate all viruses. At the beginning of the stage, there is a group of Brick Blocks surrounding three isolated exploders and a group of viruses, and clouded viruses are at the top-right side. As the stage scrolls, ten viruses inside bubbles, four half-frozen viruses and two clouded viruses are found in addition to twenty normal viruses, as well as a shell.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 9 lines are introduced.

When cleared, the rest of the first area in the world map gets cleared of viruses. Players also obtain 500 coins upon completing this stage.

Prior to version 2.0.0, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 56 to 50.
  • The row of Empty Blocks visible after the stage scrolled all the way is removed, moving all the stage objects one row above.
  • The Brick Blocks in the beginning within the second and third rows and second and third columns are changed to regular viruses.
  • A bubble virus is added in the third column in the row that is visible after the stage scrolls up once.
  • The shell visible after the stage scrolls up three times is no longer half-frozen.
  • The second and fourth Brick Blocks visible after the stage scrolls up seven times are changed to normal viruses.

The objective of this stage is to eliminate all viruses. In this stage, there are two rows of Empty Blocks with a shell in between each of them. Betweek the shells are exploders, which are between either Empty Blocks, clouded viruses or a lock block. On the left and right sides between the Brick Blocks are cages, where the yellow cages are surrounded by blue cages and contain viruses and one lock block.

  • Capsule amount is increased from 42 to 45.
  • Exploders are added in the second and eight columns in the second row, replacing Brick Blocks.

Prior to version 2.0.0, the stage layout was different. The following changes were made:

  • The floatie viruses are no longer half-frozen.
  • The middle virus is fully-frozen instead of half-frozen.
  • The viruses found in the second column, eighth column, second row and eighth row are changed to half-frozen.

The objective of this stage is to eliminate all viruses. The caged viruses in this stage spell "LOVE", with lock blocks throughout the stage. Hidden viruses are within the Brick Blocks between the blue and yellow lock blocks (5) and within the yellow cages (4).

Prior to version 2.0.0, the stage layout was different. The following changes were made:

  • The cages are moved up one row, and subsequently, the Brick Blocks in the tenth row are removed.
  • The lock blocks have their positions changed: the yellow lock block moved from the first row to the fifth row, the blue lock block moved from the sixth row and fourth column to the fifth row and the second column, the red lock block moved from the eighth row to the ninth row.
  • A hidden virus is added where the blue lock block used to be, relative to the changing position of the letters.

The objective of this stage is to eliminate all viruses. In this stage, there are three cages in the middle column surrounded by Brick Blocks, and one of the lock blocks is in one of the cages. Other viruses that are present include clouded viruses and viruses inside bubbles. A shell is located at the leftmost position in the tenth row.

Prior to version 2.0.0, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 48 to 46.
  • The Empty Blocks and the first and last Brick Blocks in the third row are changed to caged Brick Blocks.
  • The middle caged virus in the fifth row is changed to a caged lock block that corresponds to the caged Brick Blocks mentioned above.
  • Cages are removed from the fourth column and sixth column of viruses.
  • The Brick Blocks in the tenth row no longer contain hidden viruses.

The objective of this stage is to collect all virus coins. In this stage, there are four rings of cages surrounding a group of four viruses each, and each of those groups has a virus coin behind. Separating the cages are lines of viruses, with a lock block in the middle and an additional lock block in the middle of the first row. The sixth row of viruses, as well as the viruses above and below the middle lock block, have virus coins behind them.

The capsule amount is increased from 25 to 32 since version 1.3.5. Besides that, this stage is otherwise the same.

The objective of this stage is to eliminate all viruses. In this stage, there is a group of viruses in bubbles around the bottom middle part and two groups of dust viruses on its left and right. Above each group of dust viruses is a clouded floatie virus surrounded by Brick Blocks, which is partially surrounded by normal viruses. The Brick Blocks in the fourth and sixth columns contain hidden viruses.

The capsule amount is increased from 40 to 43 since version 2.0.0. Besides that, this stage is otherwise the same.

At the higher corners are exploders. The Brick Blocks adjacent to the empty spaces in the third column and seventh column are Coin Blocks, while some of the Brick Blocks contain hidden viruses.

Prior to version 1.3.5, the stage layout was different. The following changes were made:

  • Some of the Brick Blocks are removed, revealing the normal viruses for those that originally had hidden viruses.
  • Exploders are added at the corners in the first row.
  • Brick Blocks and Empty Blocks are removed in the tenth row, except for the leftmost Empty Block.
  • The Empty Blocks in the top-right are changed to Brick Blocks (except the corner, which has an exploder).
  • The Brick Blocks in the middle column between the two rows of cages are replaced by one Empty Block in the higher row.

The objective of this stage is to eliminate all viruses. In the middle, there are two columns of five viruses and one lock block, and in between are three Brick Blocks (all with hidden viruses) in a column with one space apart.

Prior to version 2.0.0, the stage layout was different. The following changes were made:

  • The middle column is changed from two Brick Blocks (both with hidden viruses) and two Empty Blocks to three Empty Blocks (all with hidden viruses) each with one space apart.

The objective of this stage is to eliminate all viruses. In this stage, fully-frozen viruses form the number 99 between two rows of viruses inside cages. Within the first, fourth, sixth and last columns, there are hidden viruses, while the other Brick Blocks do not have anything inside. A lock block is within the hole of the first "9".

Prior to version 2.0.0, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 54 to 50.
  • The Brick Blocks in the second, third, seventh and eighth columns no longer contain hidden viruses, leaving only the first, fourth, sixth and last columns with them.

The final stage is a scrolling stage that requires all viruses to be eliminated. At the beginning of the stage, there are dust viruses and clouded viruses in addition to normal viruses placed around Brick Blocks and Empty Blocks. Further up the stage are floatie viruses and hidden viruses inside Brick Blocks.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 5 lines are introduced.

Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 6. Upon completion, players earn a free Staff Ticket.

Prior to version 2.0.0, the stage layout was different. The following changes were made:

  • The row of Empty Blocks visible after the stage scrolled all the way is removed, moving all the stage objects one row above.
  • The floatie viruses in the row that appear after the stage scrolls twice are changed to normal viruses.

Within a 105 second time limit, players need to eliminate all viruses. In this stage, viruses are lined up against lines of Empty Blocks and Brick Blocks, with most of them being half-frozen. The top-left portion of viruses are enclosed by Empty Blocks.

Starting from version 1.2.0, completing this stage awards the player 500 coins.

The timer is increased 90 seconds to 105 seconds since version 2.0.0. Besides that, this stage is otherwise the same.

The timer is increased from 65 seconds to 85 seconds since version 2.0.0. Besides that, this stage is otherwise the same.

Within a 107 second time limit, players need to eliminate all viruses. In this stage, the leftmost and rightmost columns contain fully-frozen viruses, along with caged viruses between the top and bottom of each column. In the middle are clouded lock blocks surrounded by Brick Blocks and Empty Blocks, along with two groups of clouded viruses.

Completing this stage awards the player 1000 coins and a Staff Ticket (3 Hearts prior to version 1.2.0).

The timer is increased from 90 seconds to 107 seconds since version 2.0.0. Besides that, this stage is otherwise the same.

The objective of this stage is to eliminate all viruses. In this stage, there are three sections separated by two separate rows of Empty Blocks. Each section has a row or Brick Blocks with viruses between them and the row of Empty Blocks. In addition to normal viruses, the middle section contains floatie viruses and viruses inside bubbles, while the top section contains clouded viruses and dust viruses (both types also contain floatie viruses).

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 3 lines are introduced.

Completing this stage awards the player 1000 coins and Dr. Toadette.
