According to the trophy there is only one, also according to the trophy he's been practicing the skill of holding the sign for years. It is unknown why he would take so long but it could refer to him having no arms but holding the sign anyways, or a joke to give him a more in depth explanation for his trophy other than "he stopped Yoshi from passing but he ground-pounded him to get pass".

In Yoshi's Story, Pak E. Derm holds up a sign and does not let Yoshi pass. He stands in the middle of the path Yoshi is taking, and will move to the other path if Yoshi moves to the other path. To pass, Yoshi has to Ground Pound the ground, and Pak E. Derm will fall on his head, allowing Yoshi to pass. Yoshi has to act quickly because Pak E. Derm will soon flip back upright. Yoshi encounters Pak E. Derm three times throughout the first level, and he does not appear thereafter. Pak E. Derm's Message Block states, "Pak E. won’t let you by, unless you give something special a try!"
