Big Kamukamu is an optional boss fought in one of the secret chapters of Wario Land II. It is a purple, spiked fish that attacks Wario and tries to eat him, spitting him out of the arena if it succeeds in doing so. After each hit, he swims faster and becomes more enraged. After the third hit, he turns red. The fourth and final hit defeats the boss. It appears to be a boss version of the Kamukamu enemy. However, the smaller version is only found in the Game Boy version of the game, whereas the boss is present in both versions.

During the boss' first hit, it charges Wario and rebound off the edge of the screen until it reaches the top of the water, at which point Wario can ram his belly from below. During the next hit, he does not rebound off the wall, and on the third hit, he only rises halfway up the water. On the final hit, he changes to red (on Game Boy Color) and not go up past halfway. He also does not cease chomping (where other times he stops chomping and floats down) and continuously moves back and forth. Wario must carefully time his hit and hit him in the belly.
