The Manhole Hook is a special item in Paper Mario: The Origami King. It is located in a warehouse in Toad Town, and is used to unlock Graffiti Underground, allowing Mario and Olivia to travel through the sewers to reach the entrance of Peach's Castle. Before heading out to find it, Mario must rescue a Shy Guy from being eaten by a Paper Macho Goomba. To do this, he must enter a house on the west side of Toad Town. Although he is unable to open the door due to the Paper Macho Goomba blocking the other side, he can use his hammer to pop open a drawer, jump onto the drawer, and hammer the window open to get into the backyard. After Mario has rescued both this Shy Guy and his friend, who is currently stuck in the doors of a house near the sewer lid, he tells them he left it in the red brick building at the port.

After this, Mario must head back into the warehouse and hammer the shelves on the right side of the room multiple times to drop down a box, activating a Magic Circle. This allows Mario to use the 1,000-Fold Arms to pull off some of the northern wall, revealing a door taking him to a second room. Here, he must hammer the green cart to the right, allowing him to reach the Manhole Hook atop the crates. Mario can then jump out the window and head back to the sewer lid, which he can now open.

The Manhole Hook remains in Mario's inventory until he returns it to a green Toad at a house next to the sewer after clearing the green streamer. By doing so, the Toad hangs it on the wall and rewards Mario with Collectible Treasure No. 14: Mario.
