World 15 is a sunset cloud world with floating platforms in Dr. Mario World, resembling Bouncy Cloud Boomerangs from New Super Mario Bros. U. The first area is available as of 2 AM PT on Apr. 26, 2020[1], while the second area, challenge stages and special stage are available as of 2 AM PT on May. 14, 2020[2].

When an area is cleared, Paratroopas, Parabombs, and Foos appear. The viruses featured in the regular stages are red, yellow and light-blue.

In this stage, there are three columns of barriers in the middle with exploder in between, with groups of viruses mostly surrounded by Empty Blocks next to this group. The corners contain clouded viruses and Brick Blocks in the shape of a "+", where there is a hidden exploder in the middle of each group and hidden viruses in the others.

In the middle five columns, viruses are in every third row starting from the third row, while bubbles containing coins are in every third row starting from the second row.

Purple coins are unlocked in the following sequence:

  1. The 5 coins inside bubbles starting from row #2.
  2. The 5 coins inside bubbles starting from the second-revealed row.
  3. The 5 coins inside bubbles starting from the fifth-revealed row.
  4. The 5 coins inside bubbles starting from the eighth-revealed row.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 10 lines are introduced.

When cleared, the rest of the first area in the world map gets cleared of viruses. Players also obtain the Parabomb assistant upon completing this stage.

Many of the Brick Blocks contain hidden objects, which include viruses, exploders, shells, purple coins and Capsule Plus. Higher up the stage are viruses in the formation of a "V" below colorful crates, which are all below purple coins. Empty Blocks are located between the purple coins in the four topmost rows. The filled flasks in this stage will shoot out either exploders or Brick Blocks.

Purple coins are unlocked in the following sequence:

  1. The 1 coin in row #2 (before the stage scrolls).
  2. The 5 coins hidden in the Brick Blocks between the flasks in row #3 (before the stage scrolls).
  3. The 9 coins beside and above the Empty Blocks in the last four revealed rows.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 9 lines are introduced.

Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 16. Upon completion, players earn a free Staff Ticket.

The third column contains crates while the seventh column contains barriers.

Completing this stage awards the player 500 coins.

A row of exploders is revealed after the stage scrolls up five times.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 6 lines are introduced.

Completing this stage awards the player 500 coins.

  1. The 8 coins in the ninth row from left to right, one coin at a time.
  2. The 6 coins in the eighth row from right to left, one coin at a time.

  4. The 6 coins in the sixth row from right to left, one coin at a time.
  5. The 6 coins in the fifth row from left to right, one coin at a time.
  6. The 7 coins in the third row from right to left, one coin at a time for the first three coins but all four coins simultaneously for the rest.

Completing this stage awards the player 1000 coins and a Staff Ticket.

Starting with 37 capsules, the objective of this stage is to collect all coins from Coin Blocks (15), collect all purple coins (21) and collect all keys (8).

In this stage, the purple coins are placed in a checkered pattern in the second to seventh columns and third to ninth rows. In the tenth row are caged keys and two flasks. The rightmost column contain bubbles containing viruses and a lock block. The third to seventh Brick Blocks in the eighth row contain coins. The filled flasks in this stage will shoot out either exploders or shells.

Purple coins are unlocked in the following sequence:

  1. The 4 coins in the column #2.
  2. The 3 coins in the column #3.
  3. The 4 coins in the column #4.
  4. The 3 coins in the column #5.
  5. The 4 coins in the column #6.
  6. The 3 coins in the column #7.

Completing this stage awards the player 1000 coins and Dr. Lakitu.
