Similarly to the fake Bowsers in Super Mario Bros., King Totomesu attacks by trying to jump on and shoot fire at Mario. King Totomesu can be defeated by touching the switch behind him or by shooting five Superballs at him. The reward for defeating King Totomesu is 5000 points and being allowed to progress into the Muda Kingdom.

King Totomesu is the first boss faced in the Super Mario Land manga. Using Mecha Kuribō's sensors, Mario discovers he needs to hit him with five Superballs, but only manages to land four hits, before the boss swipes at him, making him lose the Superballs. Mario attempts to stomp on him, but discovers he cannot damage him this way. Avoiding another claw swipe, Mario lands on the switch and King Totomesu explodes.

In Super Mario-kun, King Totomesu appears in the last chapter of volume 3 alongside other Super Mario Land enemies and bosses. He disguises himself as Princess Peach in the Back Door room and gets rescued by Mario, Luigi, and Yoshi. The heroes then flies away with the winged Marine Pop out of the Valley of Bowser. When Bowser laughs at the heroes, confusing them, King Totomesu reveals himself. The boss quickly manages to get the upper hand, burning the heroes, who attempted to fight him with Fire Flowers but ran out. Right when King Totomesu charges at them, Hiyoihoi and the moai appear, claiming Mario is their prey, and they start fighting for him, letting the heroes walk away.
