World 2 is a desert world in Dr. Mario World. It introduces bubbles which can float upwards and Virus Coins which are background objects that require removing all non-capsule objects to collect. The second area introduces colored cages where eliminating the lock blocks of that color removes them.

When an area is cleared, Spikes and Pokeys appear. The viruses featured in the regular stages are red, blue and green.

Starting with 32 capsules, the objective of this stage is to eliminate all viruses (26).

This stage introduces Bubbles, which float toward the top of the screen and are immune to being destroyed when floating. This stage is shaped like a crown formed by normal viruses and viruses inside bubbles.

Prior to version 2.0.0, the stage layout was different. Previously, viruses inside bubbles are arranged in columns in the top two rows and rows below that, while the normal viruses are formed by an upside-down "T" with an empty middle.

Starting with 36 capsules, the objective of this stage is to eliminate all viruses (39).

This stage has a bunch of bubbles at the bottom, with viruses between blocks above.

Prior to version 2.0.0, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 35 to 36.
  • Two viruses are added in the fourth row near the middle so that there are five viruses in a row.
  • The Brick Blocks in the second row now contain hidden viruses.

Starting with 33 capsules, the objective of this stage is to eliminate all viruses (24).

This stage has Shells at the top with bubbles below, to the right. Half-frozen exploders and Brick Blocks are also found.

Prior to version 2.0.0, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 38 to 33.
  • The Empty Blocks in the first row except the first one are replaced with viruses inside bubbles.
  • The second row now contains viruses inside bubbles (the shell is intact).
  • The Empty Blocks in the third row are replaced with half-frozen Brick Blocks. Brick Blocks now occupy the other parts of the row.
  • The fourth row now contains half-frozen exploders and half-frozen Brick Blocks (the shell is intact).
  • The formation five rows below row #4 is changed around so that it occupies three rows that contain twelve viruses inside bubbles and five Brick Blocks (previous amounts were 14 and 6 respectively).

Starting with 39 capsules, the objective of this stage is to eliminate all viruses (34).

A diagonally-oriented stage, with three lines of viruses. The viruses in the middle line are half-frozen.

Starting with 37 capsules, the objective of this stage is to collect all Coin from Coin Blocks (48).

A stage that involves obtaining coins from Brick Blocks, with four columns of blocks and one row above. Exploders are located in between most of the Brick Blocks, while the leftmost and rightmost columns contain half-frozen viruses.

Prior to version 2.0.0, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 35 to 37.
  • The second and fifth Brick Blocks in the second row are replaced by exploders.

Starting with 32 capsules, the objective of this stage is to eliminate all viruses (37).

This stage has lots of viruses between Brick Blocks, including bubble viruses. There is also a shell and a Skill Charger.

Prior to version 2.1.0, the stage layout was different. The following changes were made:

  • The bubble (containing a virus) in the ninth row is removed.
  • The viruses (inside bubbles) in the seventh row are no longer covered inside bubbles.

Starting with 25 capsules, the objective of this stage is to eliminate all viruses (50). The only color available in this stage is red.

This stage is shaped like a question mark of red viruses surrounded by Brick Blocks. The Brick Blocks may contain viruses or exploders.

Prior to March 18, 2021, the stage layout was different. The stage was initially one with lots of bubble viruses, along with an exploder in a bubble, with the objective of eliminating all (37) viruses. The capsule amount is also reduced from 36 to 25.

Starting with 35 capsules, the objective of this stage is to eliminate all viruses (24).

A stage shaped like a percent sign, made of ice viruses and brick blocks. Each brick block contains a red virus.

Prior to version 2.0.0, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 45 to 40.
  • The second and sixth Brick Blocks along the diagonal line of Brick Blocks no longer contain hidden viruses.

Prior to version 2.1.0, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 40 to 42.
  • The last Brick Block in the third row and the first Brick Block in the ninth row are removed. The red viruses that were previously hidden are moved to be between the Brick Blocks in the third and eighth rows.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • This stage is no longer labeled as "Hard".
  • Capsule amount is reduced from 42 to 35.
  • The viruses in the middle of the two circles of Brick Blocks are changed to exploders.
  • The Brick Blocks along the diagonal line no longer contain hidden viruses. Moreover, the two Brick Blocks that were gone in the previous update are restored.

A stage with lots of ice viruses, with short columns of Empty Blocks and a shell at the top.

The capsule amount is increased from 30 to 35 since version 2.1.0. Besides that, this stage is otherwise the same.

Starting with 35 capsules, the objective of this stage is to eliminate all viruses (22) and collect all coins from Coin Blocks (69). The only color available in this stage is blue.

In this stage, there is a diamond of blue viruses surrounded by Brick Blocks. The Brick Blocks that are not Coin Blocks either contain viruses or exploders.

Prior to version 1.3.5, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 35 to 38.
  • The middle virus at the bottom of the Brick Blocks is moved one space below.
  • The middle virus at the topmost row is removed and are replaced by two viruses diagonally below it.
  • The Brick Blocks in the middle column no longer contain any coins.

Prior to March 18, 2021, the stage layout was different. The stage was initially one with coins forming a rectangle of brick blocks with viruses surrounding it and two exploders inside, with the objective of collecting all (36) coins from Coin Blocks. The capsule amount is also reduced from 38 to 35.

Starting with 25 capsules, the objective of this stage is to eliminate all viruses (31).

This stage has bubble viruses between blocks, with lots of viruses in brick blocks at the top.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 30 to 25.
  • The group of viruses in rows #8 and #9 are moved 1 column to the left.
  • All Empty Blocks are changed to Brick Blocks, while the Empty Block and Brick Block in row #9 being removed entirely.
  • A shell is now hidden inside the Brick Block #6 in the topmost row.
  • The viruses in columns #2 and #4 are no longer hidden inside Brick Blocks.

  • Capsule amount is increased from 29 to 32.
  • Three viruses are added adjacent to the virus in the middle column.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Brick Blocks are removed from row #10, except for the ones that were initially Empty Blocks.
  • All Empty Blocks are changed to Brick Blocks
  • The viruses in the middle column are gone
  • A shell is added in the middle Brick Block in the topmost row.

Starting with 30 capsules, the objective of this stage is to eliminate all viruses (28). The only colors available in this stage are red and blue.

This stage has a column of exploders between brick blocks, with viruses between brick blocks on the sides. The blocks contain viruses and pop cannons.

The capsule amount is increased from 25 to 27 since version 2.0.0. Besides that, this stage is otherwise the same.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • This stage is no longer labeled as "Hard".
  • Capsule amount is increased from 27 to 30.
  • The available colors are changed to red and blue only.
  • All revealed viruses are changed to half-frozen.
  • All Empty Blocks are changed to Brick Blocks, with pop cannons hidden in the corners in the topmost row while the rest have hidden viruses.

  • The available colors in this stage are changed to blue and green only.

  • An exploder is added in the middle of the top-left ring, and another exploder replaced a virus in the middle of the triangle.
  • Ice is removed from this stage, instead bubbles (with viruses inside) are found.
  • A shell is added in the topmost row.
  • The first Brick Block in row #8 is replaced by a virus.
  • Some viruses are hidden inside the Brick Blocks around the triangular area.

Starting with 34 capsules, the objective of this stage is to collect all coins from Coin Blocks (15).

This stage has brick blocks in the shape of four brackets, each with a skill charger. The Brick Blocks that contain coins are the corners of each group of Brick Blocks at the corners, as well as the middle Brick Block.

The capsule amount is increased from 30 to 34 since version 2.1.0. Besides that, this stage is otherwise the same.

In this stage, both sides have a column of bubble viruses in a set of many empty blocks, with exploders next to them. Several viruses are present at the top.

The capsule amount is increased from 25 to 28 since version 2.1.0. Besides that, this stage is otherwise the same.

A diagonally-oriented stage with regular, ice, and bubble viruses between bricks.

  • The Empty Blocks in row #9 are removed.
  • An exploder replaced the middle virus in the top-left group of 9 viruses.
  • The viruses in column #6 are inside bubbles instead of being half-frozen.

Starting with 38 capsules, the objective of this stage is to eliminate all viruses (43). The only colors available in this stage are red and blue.

The last stage of the first area. It has half-frozen viruses between brick blocks, with more of the stage coming downward after enough is cleared. The Brick Blocks in the first column and sixth column on the third row and the second-revealed row contain hidden viruses. The Brick Blocks in the third column and eighth column on the fourth-revealed row and most of the first row also contain hidden viruses. Shells are hidden in the middle column for the first row, fourth row and second-revealed row. Exploders are hidden inside Brick Block #1 in row #4 (before the stage scrolls) and Brick Block #2 in the second-revealed row.

This stage will scroll for one line each time enough objects are cleared and will stop after 5 lines are introduced.

Completing this stage awards the player with 500 coins.

Prior to version 2.0.0, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 47 to 45.
  • The first and third Brick Blocks in the bottom-most row are removed.
  • The third and fifth Brick Blocks in the row that appears after the stage scrolls four times no longer contain hidden viruses.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 45 to 38.
  • The available colors are changed to red and blue only.
  • Row #6 (before the stage scrolls) is removed entirely.
  • Bubbles are removed, where the revealed viruses are half-frozen including the viruses in the topmost row that were normal viruses initially.
  • Viruses are added in the third space in row #5 (replacing a Brick Block), directly above the viruses in row #4 (before the stage scrolls), the second-revealed row and the two rightmost spaces in the fourth-revealed row.
  • The Brick Blocks that didn't have hidden viruses in row #1 now have them.
  • Exploders are now hidden inside Brick Block #1 in row #4 (before the stage scrolls) and Brick Block #2 in the second-revealed row, where they initially didn't contain anything.
  • The row of Empty Blocks in the topmost row is removed, rendering the stage to scroll for only 5 rows instead of 6.

This stage introduces lock blocks. When activated, they expose objects in the cages of the same color. The stage is made up of five columns of caged viruses, with empty blocks in-between.

Starting with 30 capsules, the objective of this stage is to collect all virus coins (12).

This stage introduces Virus Coins, which are hidden behind objects and will require removal of them all to reveal them fully.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Exploders are added in the ring of red and blue viruses and a shell is added in between the green viruses in row #7.
  • The row of Brick Blocks in row #4 is replaced with a shell and a row of 7 viruses each covering a virus coin.
  • The virus coin removed behind green viruses in rows #6 to #8.
  • The group of viruses in the middle of the three topmost rows have one large virus coin instead of 4 smaller ones
  • The left and right virus coins within the three topmost rows have increased in size, each changing from 2x2 to 3x3.

  • The available colors are changed to red and blue only. As a result, the green cages and lock block are changed to red, while the original red lock block is replaced by a virus.

  • Capsule amount is reduced from 40 to 30.
  • The objective of this stage is changed to require eliminating all viruses in addition to collecting all virus coins.
  • The available colors are changed to red and green only. As a result, the blue lock block and cages are changed to red.
  • The Brick Block directly below the lock block now has a hidden exploder.

  • The lock block is replaced with a shell, and as a result, the viruses are no longer caged.

A stage with many sections divided by brick blocks. Each section has a set of viruses with a hidden coin.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 40 to 35.
  • The objective of this stage is changed to require eliminating all viruses in addition to collecting all virus coins.
  • Viruses are added in the left and right edges of row #4.
  • Pop cannons are added in the left and right edges of row #8.

The lower half of the stage has five areas divided by empty blocks, each having a column of bubble viruses, blocked off by a row of brick blocks above. The upper half has sets of brick blocks with viruses surrounding the side ones.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 25 to 30.
  • A shell is added in the middle Brick Block in row #5.
  • An exploder is added in the middle Brick Block in row #2.
  • Brick Blocks are added to fill the gap in row #1.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • The two spaces below the green shell in row #5 have blue viruses, replacing anything that used to be there.

Starting with 32 capsules, the objective of this stage is to eliminate all viruses (44) and destroy all Brick Blocks (36). The only color available in this stage is red.

In this stage, the group of viruses resemble a pinwheel with an exploder in the middle, where they are surrounded by Brick Blocks.

Prior to version 2.0.0, the stage layout was different. The following changes were made:

  • The Empty Blocks in the fifth row are removed.
  • The topmost viruses inside bubbles in the third and seventh columns are shifted to the second and eighth columns respectively.

Prior to March 18, 2021, the stage layout was different. The stage was initially one with many bubble viruses and brick blocks, with the objective of eliminating all (28) viruses. The capsule amount is also reduced from 40 to 32.

Starting with 42 capsules, the objective of this stage is to eliminate all viruses (37).

The last stage of the second area, with more of the stage coming after enough is cleared. The first part has sets of bubble viruses between many brick blocks, with a column containing shells and viruses on the right side. More bubbles with viruses are revealed in the next two rows, and the next two rows have normal viruses between brick blocks.

This stage will scroll for one line each time enough objects are cleared and will stop after 4 lines are introduced.

Completing this stage earns the player a free Staff Ticket.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 50 to 42.
  • The bubble viruses in rows #4 and #5 are moved to rows #5 and #6, where they are in between Brick Blocks while the left edges of both rows are empty.
  • Brick Blocks occupy the rest of the row with the shell in row #4.
  • Viruses are changed from half-frozen to being inside bubbles for the first two revealed rows.
  • The row of Empty Blocks are removed from the topmost row, rendering the stage with only 4 rows to scroll through.

This stage has two sets of many columns of viruses, divided by caged brick blocks that must be unlocked.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • The available colors are changed to light-blue and pink only.
  • The contents of the third cage in row #4 is changed from a Brick Block to a shell.

Starting from version 1.2.0, completing this stage awards the player 500 coins.

Within an 80 second time limit, the objective of this stage is to eliminate all viruses (51). The colors available in this stage are green, light-blue, and pink.

This stage has many viruses surrounding small sets of brick blocks.

The timer is increased from 60 to 80 since version 2.0.0. Besides that, this stage is otherwise the same.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • All objects within row #9 are removed, removing 5 viruses in the process (1 inside a Brick Block).

Starting from version 1.2.0, completing this stage awards the player 500 coins.

Within a 120 second time limit, the objective of this stage is to eliminate all viruses (60). The colors available in this stage are green, light-blue, and pink.

This stage has long columns of viruses on the sides, with rows of pink viruses and rows of caged viruses and blocks throughout the middle.

The timer is increased from 90 to 120 since version 2.0.0. Besides that, this stage is otherwise the same.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • The cages in rows #7 and #8 are removed.
  • The lock blocks are no longer inside bubbles. Also, their positions are switched with the Brick Blocks two rows above them.
  • Exploders replace the viruses in the first and last cages in rows #3 and #5.

Completing this stage awards the player 1000 coins and a Staff Ticket (3 Hearts prior to version 1.2.0).

Starting with 38 capsules, the objective of this stage is to eliminate all viruses (56).

This stage has several bubble viruses, with two exploders and shells in the middle, along with caged shells and a caged exploder in the top left corner. When enough is cleared, the rest of the stage comes in, made entirely out of brick blocks with a lock block in the middle.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 8 lines are introduced.

Completing this stage awards the player 1000 coins, and as of Version 1.2.0, Dr. Yoshi is also awarded.

Prior to version 2.2.0, the stage layout was different. The following changes were being made since:

  • The row of Empty Blocks and a row of Brick Blocks are removed from the two topmost rows, in addition to the objects moving up by another row, moving the objects up by three rows total.
