In this level the player must hit the first of a row of [[Brick Block]]s to release a [[? Switch]] that will make five doors appear when hit. The top door contains one [[Boo]], what appears to be a solid fog, 14 coins, a [[Star Coin]] in the bottom left corner, and a ? Switch you must hit to to activate the exit door. Mario or Luigi can navigate through the fog by ground-pounding to temporarily clear some fog directly beneath them. The middle-left door contains 3 pumpkin enemies, a Star Coin and another solid fog - this time moving in waves. The Star Coin can be reached if the player jumps at the top of one of these waves. The middle right door contains a Boo hidden inside a Brick Block and a group of harmless floating hands that point to the invisible sliding box that when hit contains a power-up. If you are [[Small Mario]], this will be a Red Mushroom. If you are [[Super Mario]] or above, it will be a [[Fire Flower]]. The bottom left door contains a ? Switch, 3 Boos and 2 columns of coins that Mario or Luigi collect instantly upon falling out of the entrance door. Once the ? Switch is hit, the player must quickly make their way to the opposite side of the room to get out of the now activated door before it disappears. The bottom right door leads to the exit. Inside is another group of pointing hands that direct you to the moving box that contains your ? Switch. When hit, 2 moving platforms appear for a short time that (once you bounce up to them) bring you up to the next door. In this new room, you hit a [[P Switch]] that for a short time turn the surrounding coins into solid platforms. Here you can take a shortcut to exit the level: going ''left'' until you see an L-shaped platform with a brick block that is shiny (unlike the others). If you hit it from below it will create a vine to a hidden door, which leads to a red flagpole that ends the level. Alternatively you can leave the room the standard way, running to the ''right'' very quickly. The third coin is in a high platform at the top right corner of the room, accessible only to the quickest players. Once you leave the exit door you are at the black flagpole that ends the level.

World 7 is the seventh world in New Super Mario Bros. It takes place in the sky, and is made up entirely of clouds, though some levels take place in overworld or mushrooms. To get to this world, the player must defeat Petey Piranha as Mini Mario, or by taking the cannon in World 4. The enemies first encountered in World 7 include Banzai Bills, Flame Chomps, Squigglers, Burners, and Ball 'N' Chains. The boss is Lakithunder, a Lakitu riding a thunder cloud. It has the most exits out of any world in the game, containing a total of fifteen exits. This is also the only world where the player needs to find at least one alternate exit to finish the world, making it similar to the Forest of Illusion from Super Mario World.
