Just then, Gahaha Daiō arrives on a flying vehicle, holding two giant beakers with Viruses, each with Mario and Bowser's names on. Gahaha Daiō challenges Mario to a VS. game of ''Dr. Mario'', but he refuses. Having anticipated that, Gahaha Daiō asks Peach to check the party ball that is hanging beside. With a device, Peach registers a high concentration of Viruses: if Mario refuses the challenge or loses it, the party ball will open, releasing a bunch of Gahaha Viruses. Dr. Mario is the fifth volume of Kodansha's Super Mario manga published under the KC Deluxe label.
The three stories included are directly based on Dr. Mario.
While the character of Dr. Mario will be later introduced in the series as his own separate character, in this volume, he is still Mario in a doctor outfit.

Chapter 1
At Bowser's Castle in the Dark Land, the Koopa king is finishing his newest plan to take over the Mushroom Kingdom: the Gahaha Virus.
At the Mushroom Kingdom, the World Ikebana Expo is taking place. While Princess Peach (the judge and host of the event) is trying to disqualify Princess Daisy's floral arrangement of her and Mario's dream wedding, a woman in Dutch attires presents to the princesses her flowers (some Piranha Plants) whose aroma is capable to relax. After smelling it, the two starts to laugh and while warmly hugging each other. Confused and worried, Mario demands the woman to tell what did she do. She unmasks, and reveals to be Bowser, now going as Gahaha Daiō, and smashes the Piranha Plants' pots on the ground, making them grow into big Dokudoku Pakkun Flower, and explains that his plants are capable of release a special germ he is created, the Gahaha Virus, capable of making everyone laugh uncontrollably. As the plants blows the germs around, Mario quickly covers his nose and mouth, while the rest of the Toads and Goombas at the event start to laugh. Mario escapes, but the plants disperse the germ all around the Mushroom Kingdom.
Mario manages to reach his home safely and starts to inspect a specimen of Bowser's virus he captured and put it in a Petri dish with his lab equipment. Unfortunately, the Virus is resistant to every known cure and antibiotic. On the news, the virus has expanded in all the Mushroom Kingdom, but it seems that a region in the Kinoko Yama has been spared. Hopefull that the place contains a solution for this virus, Mario goes to take the Sky Pop, finding the two princesses giggling while they decorate the plane with flowers, so he takes the two with him. At the Kinoko Yama, Mario is glad to see the Mountain Toad calm and healthy and asks them how did they prevent the virus infection, but none of them is even aware of the virus pandemic.
An old lady approach Mario and give him an apple, telling him that by eating it he will know the reason of their immunity. Mario ingenuously accepts, but the princesses rush to him, wanting to eat the fruit. Mario flinches and the apple falls, splitting in two, and the princesses eat them. Just then, the village chief, warns Mario that the old lady is not one of them. The lady is in fact Gahaha Daiō in disguise, and the apple was filled to the brim with Gahaha Viruses ten times more powerful, dooming the infected to die of laughter. Gahaha Daiō then sics the Dokudoku Pakkun Flower to the villagers, and everyone runs into a gorge. The exit result blocked by a giant glowing Mushroom, to which the Piranha Plants seems repelled by. Passing through a hidden tunnel that passes under the mushroom, emerging from the opposite side of it, the village chief explains that the mushroom is their protector deity: the Miracle Kinoko.
Meanwhile, the princesses are about to have a seizure, so the village chief make them eat a piece of the mushroom, curing them and saving them from death. Having finally found a cure, Mario dons his lab coat and fills some capsules with pieces of the Miracle Kinoko. Armed with that, Mario and the princesses emerges from behind the mushroom. The Dokudoku Pakkun Flower lunge at them, but the three throws the capsules in their mouth and the Piranha Plants crumble into pieces, falling over Gahaha Daiō. The heroes then proceed to fly on the Sky Pop and drop capsules around the Kingdom. The sole energy of the Miracle Kinoko is enough to disperse the virus and vanquish it from everybody.
By nighttime, the news reveal that the virus was eradicated and, after the umpteenth argument with Peach, Daisy has returned to Sarasaland. At the Mario Bros.' house, everyone are getting ready to sleep, while the examined Virus in the Petri dish, has finally managed to get itself free.
The chapter ends with Dr. Mario asking the readers if they played the game, also adding that both the Famicom and Game Boy versions are fun.
Chapter 2
At sunset, the Gahaha Virus sneaks out of the house and meet with Gahaha Daiō, who gives it triple the power, causing the germ to mutate in three colorful Virus. The three sneak back into the house and, after being caught by an awakening Luigi, they dive into his mouth, mistaking him for Mario. Right then, the real Mario walks into the room, criticizing Luigi for taking his nightcap, only to then noticing that his brother has been infected by a strong version of the Gahaha disease, causing him not only to laugh but to bounce around. He takes the remains of the Miracle Kinoko and administrates them to Luigi, but this time the Virus are immune to it. Worried, Mario takes it to the Mushroom Kingdom Hospital.
After Peach arrives, the doctor, Kuribō, informs Mario that there is nothing they can do. On the hospital screen, Gahaha Daiō appears to mock Mario and warn him that the Viruses will kill Luigi in 6 hours, unless the Princess Peach marries him. In response, Peach tosses a desk chair at the screen (hurting Bowser, who was inside the box screen). With only five hours remaining, Mario and Peach dons their doctor and nurse outfit and take the Marine Pop to use its miniaturizing function and enter Luigi body to directly fight the Viruses.
After localizing the three Viruses, Mario first tries to bomb them down, noticing again how the Miracle Kinoko capsules (used as torpedos) are inefficient, then chase them to the stomach, where a bunch of normal Gahaha Viruses are swarming in. Due to the cramming, the Marine Pop glass cracks. With a walkie-talkie, Mario asks Kuribō for a hand. While running around, Kuribō trips onto the table, and the pepper he was using for his ramen falls on Luigi, who sneezes the Marine Pop out. Once again, Gahaha Daiō appear on the hospital screen, explaining he separated the Virus into three right to make the capsules useless and invites Peach to reconsider his marriage offer, only to receive a table through the screen.
After studing in the lab, Peach comes with a solution: if Bowser split the Virus, maybe the cure needs to be split and decomposed as well. Doing so, Dr. Mario creates six new capsules. With three hours left for Luigi, the due jumps in the repaired Marine and goes back in. Back in the stomach, Mario and Peach shoots the capsules at the small viruses in the gorge, but they repel them. Without the direct contact, it is impossible for the cure to kill them. The Gahaha Viruses mock them, infuriating Peach, who shoots a barrage of capsules, who end up clogging the cavities in the swarm. Everything seems lost, with Luigi having a stomach-ache for the clogging and the Viruses reminding that there are only two hours left.
Luigi starts to bounce around from the pain, scrambling the content of his stomach. This causes four yellow viruses to align together and to disappear. Mario notices, and with the Red Virus screaming in panic that this is their weakness, Mario and Peach starts shooting the capsules strategically to align four elements of the same color. With one hour to spare, the Viruses are vanquished and Luigi is feeling better, but before Mario and Peach can evacuate the body, Kuribō informs them that another concentration of Gahaha Viruses have appeared in another area of Luigi's body.
Chapter 3
The Viruses have now concentrated by the brain. As Mario drives the Marine Pop there, a stole away is revealed on board: Gahaha Daiō. Before he can do anything, a watermelon hits him in the head, and he falls from the ship. Looking around, a lot of food has materialized. Kuribō explains that, being nearby the brain, Luigi's thoughts can take form, and being him hungry, of course he is thinking of food. After Kuribō sacrifices his ramen to calm Luigi's hunger to avoid Dr. Mario and Peach to be overwhelmed by fruit, the two have reached the Virus lair.
Another wall of Gahaha Viruses awaits, while the three main Viruses are jumping on Luigi's nerves, causing him to burst into different emotions. While Dr. Mario yells at them to stop, Gahaha Daiō sneaks behind him and steals the capsules gun and the Marine Pop, then flies away after snatching Peach, leaving Dr. Mario alone with just a box with a limited number of capsules. Mario tries to jump at him, but the Viruses hold him down. Without any other options, Mario tosses the capsules at the Viruses, but after a while, ends up creating a mess of pills an germs.
With half an hour left for him, Luigi asks Kuribō if he can spend his last minutes reading a manga. Doing so, the manga materializes beside Mario and, as luck would have it, it is a gaming magazine which includes a special guide for Dr. Mario. Reading that, Mario discovers that he can still direct a capsule in narrows paths, as long as he turns it in time, doing so, Mario is able to recuperate. Needing to stop this, Gahaha Daiō tires to fly out of Luigi's body to destroy the magazine, and despite Peach's efforts to stop him, which only results in the Marine Pop to hits some nerves that make Luigi so angry that he knocks out Kuribō, Gahaha Daiō finds the exit and burns the comic.
It is too late, however: Mario has cleared a lot of Viruses, and only a few small groups are left. Gahaha Daiō grabs an alarm clock, showing Luigi that only 10 minutes are left and declaring that Mario will not be able to save him. The dark thoughts manifest in Luigi's brain, causing his brain to fall into the dark, but Dr. Mario needs the light to see where to drop the capsules. After tying and gagging Peach and destroying the walkie-talkie, Gahaha Daiō keeps torments Luigi about his impending doom for 8 minutes. As Mario sees Luigi's life memories passing by, he realizes that it actually might be too late.
One minute left. Kuribō regain consciousness and unties Peach, but to everyone's surprise, she accepts Gahaha Daiō's condition to marry him, which makes him very happy. However, Peach demands that first he cures Luigi, but Gahaha Daiō refuses, seeing as Luigi is just "Mario's secondary character" and he "deserves to die", then he lunges at her to kiss her. Peach dodges him, and Gahaha Daiō crashes into a wall, falling unconscious. Peach runs to Luigi, and encourages him to never give up and to trust Mario, kissing him on the cheek. Literally glowing with hope and joy, Luigi's darkness dissolves and Mario is able to eliminate the last viruses in one shot, by strategically tying some capsules. The Gahaha disease is vanquished at the very last second, the Viruses are desperate for the loss, and Luigi is saved.
Glad of that, Luigi cries of joy, and riding the tear, Mario exits his brother's body, holding the Viruses by the ears. Furious for the troubles they caused, Luigi and Peach are ready to make the three little monsters pay. Scared, the Viruses quickly flee into the first open mouth they see: their boss'. While Dr. Mario and Kuribō at Bowser's misfortune, Luigi is enjoying and treasuring the kiss he received from Peach.
