Described as "overgrown guppies,"[1] Big Mouths appear only throughout underwater areas in Stage 3, hiding in dark holes in the background, with only their eyes visible. A Big Mouth lunges with its mouth wide open at Wario when he approaches. Big Mouths cannot be defeated, though the flames of Sea Dragon Wario can cause them to retreat into their hole for several seconds, with their eyes also appearing stunned briefly.

Their design and behavior would go on to be similar to the Utsuboankō from Wario Land 4, a later installment of the Wario Land series.
