The Mushroom Kingdom Orphanage eventually appeared in The Adventures of Super Mario Bros. 3 episode "Tag Team Trouble". In the episode, the orphanage resembled a large Toad House. Princess Toadstool had given Toad a sack with million Gold Coins which he then delivered to the orphanage. Upon reaching the orphanage, lifting the heavy weight of the sack caused Toad to fall asleep in front of the orphanage after ringing the doorbell. Although the Mushroom Nanny inside the orphanage received the money, Toad wrongly assumed that it was stolen by Bowser and Cheatsy Koopa.

After a long ordeal involving a wrestling match, Mario and Luigi were able to win a million Gold Coins, which Toad brought to the orphanage. When Toad delivered the second bag, the Mushroom Nanny gave him her "double thanks" for Princess Toadstool's "double generosity". When Toad put two and two together, he fainted.

The Mushroom Kingdom Orphanage (shortened to Mushroom Orphanage in "Tag Team Trouble") is an orphanage for Mushroom children located in the Mushroom Kingdom. While first referenced by name in The Super Mario Bros. Super Show! episode "Mario and the Beanstalk," the story of its creation was outlined in "The Great Gladiator Gig." Later, in The Adventures of Super Mario Bros. 3 episode "Tag Team Trouble," it made its only onscreen appearance. The organization is run by the Mushroom Nanny and supported by donations.

In "The Great Gladiator Gig," after defeating Emperor Augustus Septemberus Octoberus Koopa, Mario and his friends organized a spaghetti dinner, allowing Princess Toadstool to collect enough money to build the Mushroom Kingdom Orphanage. In "Mario and the Beanstalk," the Mushroom Kingdom Orphanage experienced financial difficulties, needing one hundred Gold Coins to stay open, though this is resolved when King Koopa's Coins rain down upon the heroes after Luigi chops down the gigantic beanstalk holding up King Koopa's castle, providing them with more than enough Coins to save the orphanage.
