- In the place of <username>, enter the name of the user who received the reminder and/or warning.
- In the place of <template name>, enter the name of the template that the user received; whether it was a {{reminder}}, {{warning}}, or {{last warning}}.
For example, if a reminder was given to the user WikiUser, the syntax would be: {{appeal notice|WikiUser|type=reminder}}
