|align=left|This stage introduces barriers, which all turn on or off alternatively if a capsule lands without making matches. An inactive barrier can be destroyed, but active barriers cannot be destroyed. The objective of this stage is to eliminate all viruses.
|align=left|Starting with 35 capsules, the objective of this stage is to eliminate all viruses (28).

This stage introduces barrels, which require up to two hits to destroy. Destroying barrels the first time will reveal its contents, and destroying it the second time will cause the contents to be active or able to be matched. In addition to barrels, half-frozen viruses are found surrounded by barrels, each line containing a gap. All barrels contain viruses.

|align=left|Starting with 40 capsules, the objective of this stage is to eliminate all viruses (14) and collect all virus coins (5).

This stage is shaped like a four-leaf clover in the form of crates, where the barrels are in the middle that contain viruses in the undamaged barrels and an exploder in the partially-damaged one. The dusts in this stage cover exploders. Four floatie viruses are found on the outside of the border of crates, along with two more diagonally above the bottom floatie virus. The virus coins are found in the four corners and the middle within the border.

|align=left|The last stage of the first area and starting with 46 capsules, the objective of this stage is to eliminate all viruses (54), collect all coins from Coin Blocks (12) and collect all purple coins (25).

In this stage, there are three rows of barriers alternating between three rows of viruses, with one barrier in each row being the odd one out in terms of being active. A row of purple coins found partially damaged barrels is located at the top of the stage before it scrolls, with an additional five coins above that row.

On the rows above the initial seven rows of this stage, additional viruses are found in the leftmost and rightmost columns, three of them being floatie viruses of different colors. More viruses are found between Brick Blocks found on the rows #2 and #8, along with purple coins found in barrels in between those rows. At the top of the stage, four Brick Blocks are found with a caged purple coin between them. The purple coins in the undamaged barrels require other purple coins to be collected, while the single one in the top requires the other purple coins to be collected.

Purple coins are unlocked in the following sequence:
# The 14 coins inside the barrels in the first- and second-revealed rows.
# The 5 coins inside the barrels in the sixth-revealed row.
# The 5 coins inside the barrels in the fourth-revealed row.
# The coin in the middle of the eighth-revealed row.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 9 lines are introduced.

When cleared, the rest of the first area in the world map gets cleared of viruses. Players also obtain the Dry Bones assistant upon completing this stage.

Prior to version 2.0.0, the stage layout was different. The following changes were made:
* Capsule amount is increased from 37 to 43.
* The stage is shifted up by one row, causing the stage to stop scrolling after nine rows instead of eight rows.
* The leftmost and rightmost barriers in the bottom-most row are changed to Brick Blocks.

When cleared, the rest of the first area in the world map gets cleared of viruses. Players also obtain the Penguin assistant upon completing this stage.
The capsule amount is increased from 43 to 46 since March 18, 2021. Besides that, this stage is otherwise the same.

A line of regular viruses is found near the top, with the top containing a line of Empty Blocks.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 9 lines are introduced.

Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 12. Upon completion, players earn a free Staff Ticket.

Prior to version 2.0.0, the stage layout was different. The following changes were made:
* Capsule amount is increased from 44 to 46.
* The unscrolled part of the stage is shifted up by one row, removing the topmost row that consisted of two barrels containing viruses and five Brick Blocks.
* Brick Blocks are added between the barrels in the topmost row before the stage scrolls.
* The ice fans in the row visible after the stage scrolls up eight times are changed to become barrels that contain viruses. A virus is added to the middle of that row (previously it contained nothing).
* An ice fan is added to the middle of the row visible after the stage scrolls up nine times, replacing a virus.

Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 11. Upon completion, players earn a free Staff Ticket that lets them unlock either a new doctor, a new assistant, or leveling up an existing doctor or assistant.
The capsule amount is increased from 46 to 50 since version 2.1.0. Besides that, this stage is otherwise the same.

Dr. Fire Peach

Starting with 35 capsules, the objective of this stage is to eliminate all viruses (28).
