If the player comes back to the Lost Kingdom after repairing the Odyssey (before defeating Bowser in the Moon Kingdom) and attempts to go past the 2 wooden posts, Klepto will take away Cappy for the second time, and Mario must defeat him again near the Swamp Hill checkpoint, which is why Cappy could be taken a third time instead of a second time in the Klepto Lava Pit area.

- I certainly remember it appearing outside multiple times myself. Given that there's no reason to advance in the game until one has collected every moon that can be attained at that point, the above playing method is a likely scenario for a player to do.
