How do you unlock this level? Get a perfect score in every level? Sprite of the Ruby Star in Paper Mario: The Thousand-Year Door CrystalYoshi Yoshi Egg Sprite.png

In the original game, yes. In the remake on the GBA, the Secret levels give you an extra 100 points in each world after you beat the game (6-8), so you only need 800/900 points (though you can get in before beating the game on the GBA too by being perfect).
