It is based on the microgame [[Right in the Eye]], from ''[[WarioWare, Inc.: Mega Microgame$!]]'', with the revised touch controls from ''WarioWare Gold''. As with Right in the Eye, the objective is to thread maneuver a hand holding a thread so that the thread goes through the eyes of the needles without touching the needle outside of the eyes.

The main differences between Right in the Eye is that Sew Tricky is endless, and each successfully threaded needle will count towards the player's score, as well as no time limit, therefore the only way to fail is to have the thread touch the needle. In later stages, needles may travel up and down, sometimes with variable speeds and patterns, making it more difficult to thread. Every 20th needle is a black needle with a magenta outline that is stationary, but with a very narrow eye. The background and ambience changes after the player achieves a score of 20, 40, 60, and 80.
