It is given to [[Mario]] by a "[[Rawk Hawk|fan]]." If Mario chooses to eat the cake, his active [[party member|partner]] eats a portion of it first. Before Mario can have the remaining portion, that partner starts to feel sick, and Mario must face his next fight alone, after which his partner recovers. Before Mario fights with [[Rawk Hawk]], he reveals that he sent Mario and his partner the Poisoned Cake. If Mario decides not to eat the cake (though his partner will say otherwise), [[Shellshock]] eats it instead and thus is the one to be poisoned.


The Poisoned Cake largely has the same role in the [[Paper Mario: The Thousand-Year Door (Nintendo Switch)|Nintendo Switch remake]], although [[Goombella]]'s [[Tattle]] on [[Prince Mush]] during the Exhibition Match implies that Rawk Hawk used it against Prince Mush in a similar manner for the same reason. Also in the remake, the Poisoned Cake is mentioned as an incorrect option for one of [[the Creepy Steeple Boo]]'s quiz questions.
