The objective of the game is to change the character's facial elements (nose, mouth, ears, etc.) to distorted positions accurately as possible. The player who has the most accurate distorted positions wins.

The minigame is named after "{{wp|Rhytidectomy|facelift}}", and is based specifically on the title screen of ''[[Super Mario 64]]''. Furthermore, in the ''Mario Party'' version only, the background texture parodies the ''Super Mario 64'' title screen and reads ''Super Bowser 64''. In ''Mario Party'', ''Mario Party: The Top 100'', and ''Mario Party Superstars'', each player tries to distort [[Bowser]]'s facial features in split-screen gameplay. In ''Mario Party 2'', the face can be either [[Mario's face|that]] of [[Mario]] or of any of the other five player characters, being [[Luigi]], [[Princess Peach]], [[Yoshi]], [[Wario]], or [[Donkey Kong]], and their facial features change after a star flies above them. On [[Mini-Game Island]] in ''Mario Party'', the minigame requires at least 90 [[point]]s to clear.
