The fake Wario is the seventh boss in the NES version of Wario's Woods. He resembles Wario and has the power to make one whole row of mixed enemies fall. After defeating the boss, Wario pretends to admit defeat, but quickly informs Toad that the Wario he defeated was a fake one, and the real one is "ready to rumble!" Toad must then clear thirty more rounds and defeat Boom and Seizer again before fighting the real Wario.

In Super Mario-kun Volume 13, in the second to last chapter of the Wario's Woods arc, Mario and his party arrives at Wario's base, but when capturing Wario, they discover to have five of him. Looking closely, only one has the same face as the normal Wario, with the others being a Squeak, a Spud, a Beaker, and a Spook. However, Mario quickly realizes that even the fifth one is an impostor, a Fuzz. The monsters then come together to form a revolving cylinder formed by multiple Wario's faces. The Fake Wario then put the heroes inside a log, putting them through a trap functioning similar to a normal Wario's Woods level, but Wanda and the fairies manage to free them, and then destroy the Fake Wario monster by dropping Bomb Mario onto it.
