They appear only during the final battle where they are fired from Bowser Jr.'s ship, and upon descending on Mario's level, they start chasing him if they spot him. At first, only two are fired, but as Bowser's bathtub is damaged, eight are fired at once.

Guided Bullet Bills come in black and blue varieties. The black ones have the same appearance as the normal Bullet Bills in Super Mario Sunshine, but their noses blink red like Purple Bullet Bills. The blue ones are shot rarely and one at a time, their noses do not blink, and they will let out a 1-Up Mushroom or a Water Bottle if stomped on.

There are inconsistencies regarding how the Guided Bullet Bills are classified. Japanese sources acknowledge the blue variants but do not differentiate the black ones here from the ones elsewhere, contrary to English sources where both colors are considered the same type of Bullet Bill. The Prima Games guide calls them special Purple Bullet Bills, which describes their function as a subset of the homing type encountered earlier in the game.
