This level starts with a staircase leading upwards, with 10 [[Boo Buddies]] in circles. when the player goes down, there will be an Eerie as well as a [[Big Boo]]. The player can also find a blue [[P-Switch]] and a trampoline on top of a reverse-coin block. The player must make it through the first door on the right.

Once through that door, Mario or Luigi will encounter many doors that will lead him to exits for the level. There will be a power-up question mark, that gives the player a [[Mushroom]] if small size, or a [[Fire Flower]], if larger. The player must head left to find a [[P-Switch]] after running through some ghosts to access these exit doors. If the player presses the switch, there will be three question marks appearing in the middle of the map, which gives access to a stem that leads up to the secret blue door. There is also another secret blue door, guarded by a set of coins. This door will lead to the goal of this level, which leads underground to [[Donut Secret 2]]. If Mario or Luigi make their way up the stem to find the other blue secret door, it takes them to the Big Blue Boss.

To defeat the Big Blue Boo, the player must pick up the grab blocks that make up the floor and kick them at the Big Blue Boo, without taking the floor away in the process. This level is the home of the Big Blue Boo, the miniboss who protects the Star Road. If the Mario Bros. beat this boss, they'll have access to the Star World.

Overview

This level consists of two main rooms, a boss room, and a hallway used to reset the second room which the player has 400 seconds on the Timer to clear. This level contains no Dragon Coins.

In the first room, the player will have to avoid 10 Boo Buddies floating in circles. After the Boo Buddies, the player will encounter an Eerie and a Big Boo. If the player activates the P-Switch, the Used Block will turn into a Coin, and the spring that was on top of it will fall down, allowing the player to jump over the big boo easily.

The second room consists of more Boo Buddies moving in a circular motion, and some that will chase Mario or Luigi. At the beginning of the room the player can find a ? Block that contains a Mushroom if small, or Fire Flower if large. There is also a P-Switch at the far left end of the room which the player must use to find either the normal or secret exit.

Exits

The door to the normal exit can be found in the second room. To access it, the player should collect the 5 coins that make the outline of a door about halfway through. Then, the player will need to carry the P-Switch back to this spot and stomp it, making a door appear. The Giant Gate can be found through this door, and leads to Donut Secret 2.

The door to the secret exit can be found in the second room as well. To access it, the player should carry the P-Switch to the area with the door floating near Turning Blocks, activate it, and then hit the middle Turning Block to make a vine go up to a hidden ledge. The secret exit door is up there. An alternate strategy to this is to fly up there with Cape Mario. Inside this door is a mini-boss, which will fly around and try to hit the player. To defeat it, the player will need to throw Grab Blocks at it three times. The player can use the Star Road this activates to go to Star World 1.
