There are eight levels in this world, including a tower and castle that are guarded by Iggy Koopa, and a Ghost House. It also has a total of nine exits. There is also a Warp Cannon that sends the player to World 8. The enemies introduced in World 5 include Stalking Piranha Plants, River Piranha Plants, Wigglers, Big Wigglers, Bramballs, Circling Boo Buddies, Mini Goombas, and Swoops. World 5 features one Toad House of every color; red, green, and yellow. On the map, there are three Stalking Piranha Plants that act as the world's Enemy Course; this is the most that occur in any of the game's worlds. Once Iggy is defeated in the castle, the bramble patches disappear completely.

This level has Mario travelling across the toxic liquid on a wooden plank. Creatures including piranha plants, goombas, koopa-troopa and bob-ombs will land on it, causing the numders on it to increase from 0 up until 5 in which at which it turns red and stops. To make the plank start again, the player has to remove the enemies off of the board so that there are less than five.

World 5 Ghost House
The third ghost house in the game.  Common ghost house enemies can be found, including boos. It is possible to find a secret exit that will take Mario and company to the world 5 cannon.  This cannon shoots Mario to world 8, Bowser's Volcano.

5-5
This level has the player jumping across the screen on the backs of giant green mantarays. For the most of this level, the player has to avoid bullet bills and can't touch the ground as they're in the sky. The player is also given Propeller boxes which become useful as a lot of the level is also covered in clouds.

Mario and Co. start on a plattform above four plattforms on which the Stalking Piranha Plants walk. There are moving platforms directly over them on which the players can jump on to collect the Toad Balloons. They have to be careful, though, as the Stalking Piranha Plants can still reach them with their head when they stretch it out. All the player has to do is jump over them.
