They only put 2 or three in SM3SL and a LOT more in NSMB2

In SM3DL, they had four, actually, counting the one in the golden Mystery Box. NSMB2 had the similar [[Gold Block]], and they're a bit more common in 3D World, I think.

Firstly, is "Question Box" the official name? If not, in [http://www.youtube.com/watch?v=dpIwcblsO_k this video] about 3D World, the name for it is "Koin Bokkusu", which means "Coin Box". Could this be the official name? {{user:Mario jc/sig}}
