This level begins with a Roulette Block and some Elevators that all go up when touched. They serve as a platform. A Goomba falls onto the Elevators and Mario must avoid it. After a while, two more will fall on. Then three Tail Goombas will fall on. After a while, the Elevators move right, with a wide block Mario can jump over, but forces the Goombas to fall off. The wide block has a Coin Block above it. After a while, the Elevators go up again. Two Goomba Towers and a Koopa Troopa fall on, but the Elevators go right again, with another wide block forcing the enemies to fall off. A Question Block is above the wide block, but there is a Poison Mushroom in it. The Elevators go up again.
There are three Venus Fire Traps and a Moneybag. Four Elevators stop. Three Bob-ombs fall on. Spiked balls roll on the Elevators, and four more Elevators stop. After the spiked balls, there are some spiked walls. Two Prongos fall on. When the Elevators nearly have passed the spiked walls, two more Prongos fall on. Two more Elevators stop. There are three Magikoopas to try and hurt Mario. Above is a Warp Box, which takes Mario to the Goal Pole.

The level is a Ghost House which is similar in appearance to World 4-4. Mario or Luigi must avoid the Cosmic Clone during the entire level. The player starts on a large platform with one Boo on it. Reaching the end will cause a set of invisible platforms to appear. This leads to a bridge of Donut Lifts which in turn lead to pair of invisible platforms with a Warp Box. This takes the player to another set of invisible platforms, upon which are Peepas and two Boos. There is then a Donut Lift and two more invisible blocks, and then a large octagonal block with five Question Blocks, one of which contains a Star, and Boos that appear upon reaching them, one being a Tail Boo. It is here that the path diverts into four: the top right path leads to the Warp Box, which leads to a series of invisible platforms that appear when approached. This path leads to four yellow platforms and a large platform with the door leading to the Flagpole.

*Star Medal 2: The Star Medal is sighted above an invisible platform; however, it will then fly away to the octagonal platform and will stop in midair behind a set of Question Blocks.
*Star Medal 3: The third Star Medal is above another floating invisible platform, near a wall.

This level is reminiscent to World 6-1, and the earlier Special 1-4. The level starts with Mario on a grassy platform, with a bridge beneath it containing two Flophoppers. After crossing a second bridge, there will be another bridge that is raised; it is at this point that Bullet Bills (and some Tail Bullet Bills) will be automatically shot at Mario. Beside the bridge is a grass platform. After leaping onto another bridge next to but higher than the previous one, Banzai Bills will be launched. Once the player crosses another bridge with Bullet Bills being shot, Mario will once again have to escape the Banzai Bill by crossing several grassy rocks. Another bridge will then take the player to more bridges, with a Boomerang Bro. and a Venus Fire Trap on it. Afterward, the player will have to cross a grass-covered platform to reach a rock formation in the form of a staircase leading to the flagpole.
====Star Medals==== | |||
*Star Medal 1: The first Star Medal is in the cavern behind the waterfall, where the Mystery Box in World 6-1 would be.
*Star Medal 2: Between two grass-covered rock spires, the Star Medal can be seen.
*Star Medal 3: The Star Medal is visible beneath the "staircase" leading to the flagpole. It can be obtained as Boomerang Mario.
===Special 4-4=== | |||
====Layout==== | |||
| |||
The level is based on World 1-3. The player only has thirty seconds to complete the level. Instead of a Koopa Troopa, the Venus Fire Trapss, the Biddybids, the Para-Biddybids, and the Pirhana Plants, there are Goombas, but defeating the Goombas scattered throughout the level will provide ten more seconds. The player begins the level by automatically stomping one such Goomba, on a rock platform. After descending one large wooden platform, he will encounter a Switchboard, which will take him to another rock platform. This will lead to another wooden platform to descend, followed by one large drop onto another rock platform and a Switchboard. This takes Mario to a group of four Wooden Blocks, followed by another platform consisting of two Wooden Blocks, followed by a rock platform with the flagpole.
====Star Medals==== | |||
*Star Medal 1: The medal is in the exact location of the Mystery Box from World 1-3.
*Star Medal 2: Right above the lone Question Block in the level is the Star Medal.
*Star Medal 3: The third Star Medal is floating above and beside the second set of Wooden Blocks near the end of the level. It can be obtained as Boomerang Mario.
===Special 4-5=== | |||
====Layout==== | |||
| |||
This level is similar to World 3-3. However, the Donut Lifts are replaced with rock-like platforms that crumble and fall under Mario's weight, and the enemies are Ghost House-based enemies and Dry Bones. Mario starts the level on a large group of platforms, and crosses to another group when he leaps over a set of Donut Lift-style rocks. After crossing several more rock lifts and platforms, the next platform will have a Question Block with a Star in it, followed by a series of rock Donut Lifts with Boos and Peepas. Afterward, the next platform will have the midway flag on it. The next area involves wall jumping up a pair of platforms, followed by a fall to a group of Donut Lifts with a Big Boo. A platform followed by another set of Donut Lifts take the player to a Warp Box leading to another set of rock Donut Lifts and the flagpole on a nearby platform.
====Star Medals==== | |||
*Star Medal 1: Above the row of blocks near the beginning and behind the set of platforms, the Star Medal is on another platform below.
*Star Medal 2: After the wall jumping segment, the second Star Medal can be seen floating above the lower of the two platforms.
*Star Medal 3: The third Star Medal can be seen floating above the gap between a group of rock-like Donut Lifts.
===Special 4-Airship===
====Layout==== | |||
| |||
160 Star Medals are required to unlock the level. The level starts on a small front of an airship, followed by a separated interior. After crossing this, there is a Warp Pipe leading to a room with two Boomerang Bros. and a Hammer Bro.. After defeating them, a Warp Pipe will appear, leading to another airship with a Hammer Bro. and a Blokkablok on it. The following ship has two Boomerang Bros. on it, and by using the crates and platform, which has a Blokkablok hovering over it, the player will reach the next airship, which contains the midway flag and a Warp Pipe leading to Boom Boom. The fight with him takes place in a room which has spike traps of two colors; one set of spikes will appear on one colored section seconds after the others retreat. The Warp Pipe that appears after defeating him leads to an area composed of three platforms and several Donut Lifts; upon one of the platforms is a green + Clock, and collecting it will cause Pom Pom to appear. When she is defeated, a Warp Pipe will appear that transports Mario to another airship with the flagpole.
====Star Medals==== | |||
*Star Medal 1: Right after facing the Boomerang Bros. and Hammer Bro., the airship Mario will be on next will have an Exclamation Mark Block. By using this, the player can use the resulting Platform Blocks to get to the Star Medal floating above.
*Star Medal 2: Using the second Exclamation Mark Block, the player can create a 3-Platform Block staircase to reach the second Star Medal.
*Star Medal 3: The third Exclamation Mark Block, at the very front of the airship following the second, will provide three Platform Blocks that will give access to the third medal.
