Wario starts on the railway track before the train departs, where the entrance is located just past three cargo crates with a Pirate Goom on top of each one. Two blocks containing a heart and Garlic Pot float above the second one. Shortly after entering, the train begins to move. Wario must jump over a few cargo crates and break through a set of blocks to reach the door on the other side of the room, where two Pirate Gooms are also found.

Upon heading through the door and appearing outside the train, the level starts to auto-scroll. Wario first simply has to travel across the train cars towards the left, which are occupied by D.D.s. Wario will then come across a cart that moves individually along the railway track, which he must ride to continue through the level. If Wario falls onto the track itself, he instantly loses a life. The cart moves under a long series of empty blocks, which Wario must jump along while collecting the row of coins above. Eventually, the cart will arrive at the next train car, where two D.D.s are found, as well as the door leading back inside with the save point, a heart and a Dragon Pot.

After emerging back outside, the level begins to auto-scroll again, and Wario must cross another series of cars with Pirate Gooms and many bricks he can break to access the blocks containing hearts if he has time. He will then reach another cart that travels under platforms made of empty blocks with coins above and below them. Wario must alternate between ducking under the blocks and jumping on top of them while avoiding the nearby Watches to collect them. Once the cart arrives at the train car, Wario can enter the door into a cargo room, at which point the train stops moving and the level stops auto-scrolling. After exiting the train, Wario will find two D.D.s and an idle train car on the track. The goal door can be found already opened on the other side.
